At San Jose 420 Evaluations (SJ420) we are considered the premier medical marijuana evaluation center in San Jose, CA.  The goal at SJ420 is to make sure that everyone that comes to our medical office is comfortable, treated with respect and is kept private. When you enter our office you will notice that our assistants are here to help aid you in obtaining a medical marijuana license so you can use medical marijuana legally in the state of California. After you sign in for your appointment our staff will give you some quick and easy forms to fill out so you can be seen by our doctors. Once you fill out the forms and return them you name will be called and you will go into one of our evaluation rooms to meet with a doctor.

When you meet with the doctor dont be afraid to ask any questions or concerns you have about using medical marijuana and about its benefits. The goal during the medical marijuana evaluation is to see if you qualify for medical marijuana but also so you know about the pros and cons about using marijuana compare to your current drug regimen. After the doctor qualifies you for medical marijuana you will be given a card that will allow you to purchase medical marijuana that day.

You will need to provide a proof of identity verification. Accepted methods of verification include:

• Valid California ID (DMV issued California driver’s license or Identification card)
• Valid Passport or Out of State ID with proof of California residency (utility bill, lease, car or voter registration
• Valid Passport or Out of State ID with temporary DMV form
• Expired California ID with temporary DMV form
• Valid Government-issued ID
• Valid Resident Card for California
• Temporary DMV form with any photo ID
• Military Card with proof of residency
• Any “other” ID must be Government issued
• We do not accept Birth Certificates
